This helps staff determine what you need and our clinician reviews your information for any contraindications. Completing this beforehand expedites the process of ensuring a safe and effective treatment for you. We cannot provide you services without this telehealth visit. Information on this intake form is completely confidential and HIPAA compliant. You only need one telemed visit every 6-12 months, unless there are changes indicated on your intake form. I can also take direct orders from your primary care physician if you would like!

Since IV therapy involves an injection or IV, you may feel a small sting when we insert the IV, but any discomfort should pass quickly.

Once your IV is in place, you can read, watch movies, play on your phone, or even take a quick nap. Most infusions take an hour or less. The IVs used during IV infusion therapy are exactly the same medical grade equipment that you’d find in a hospital.
